If you’re worried about an upcoming review, our qualified CHC Nurse Assessors can support you through the process.

Once an individual has been found eligible for any amount of NHS funding – be that FNC, joint funding or CHC – they must be subject to regular review. This is initially at three months and then annually thereafter. If the care needs have changed significantly at the point of review, a new Decision Support Tool should be completed – this means a reassessment of CHC eligibility. 

We see many individuals who have been found eligible for FNC in the early stages of their illness and are subsequently either not reviewed at appropriate intervals or are not referred for reassessment despite a significant increase in their needs. 

Conversely, we also see many people who have been awarded CHC funding being referred for reassessment at the point of review and having their CHC funding withdrawn, despite no significant change in their needs.   

The review process is often confusing and lacking transparency. Indeed, many families are not even aware a review has taken place until it’s too late. Having an experienced CHC Nurse Assessor at your side can make all the difference in ensuring the correct outcome. 

Our approach:

The main steps involved in our Review Advocacy Service: 

  • Asking one of our specialist CHC Nurse Assessors to review the original Decision Support Tool
  • Arranging a virtual meeting between you and our Nurse Assessor to discuss any changes to your relative’s health and social care needs since the DST was completed
  • Arranging a virtual meeting between our Nurse Assessor and the care provider to fill in any gaps
  • Arranging our Nurse Assessor’s remote attendance at the FNC/CHC Review to support you and ensure the process is robust, fair and accurate
  • Providing you with a follow-up report
