This service provides support to individuals who require advocacy support at an impending Multi-Disciplinary Team meeting (MDT) but where there is not time to offer a full Clinical Review. This service includes two hours on the phone with a CHC Nurse Assessor who will then arrange to represent you remotely at the MDT.
The main steps involved for our MDT Advocacy Service:
- Arranging a virtual meeting with one of our specialist CHC Nurse Assessors to discuss your relative’s health and social care needs
- Arranging for our nurse to attend the MDT remotely to support you and to ensure the assessment is robust, fair and procedurally accurate
*Please note, owing to strict time constraints, it will not be possible for us to obtain a copy of the care records in advance of our assessment. Instead, we shall rely on the information you provide to our Nurse Assessor. This service does not include formal advice as to eligibility, only advice and support at the MDT.
